Background
The earliest approaches to voice feminization typically relied on surgical intervention. These practices were rudimentary, carrying significant risks while achieving various rates of success. In the 1980s, speech therapy gained recognition as an effective, non-surgical approach to voice feminization. In the past decade, with more information available to us than ever before, specialized speech-language pathologists are better equipped than ever to guide transgender individuals to reaching their desired voices.
Purpose and Goals
The goal of voice feminization therapy is to develop and master a voice that feels and sounds genuine and like a true expression of one’s self. The speech-language pathologist (SLP) and client work together to create goals to progress to this voice, in alignment with the needs of each individual. Once strategies are trained in the therapy session, we target the development of the skills both in and outside of sessions.
Limitations of HRT and Surgery
Hormone replacement therapy (HRT) for MTF and some NB individuals involves the use of estrogen, testosterone blockers, and sometimes progesterone. Unlike other forms of HRT, these hormones do not usually create significant changes in the vocal tract. Individuals that use this type of HRT require dedicated vocal training in order to develop and sustain the changes necessary for a feminine voice that will allow them to express themselves authentically.
The primary surgical option for transgender people who seek voice feminization is the Wendler’s glottoplasty. This procedure involves removing a portion of the vocal cords, shortening their length and increasing tension, which results in a higher-pitched voice. Unfortunately, access to one’s full pitch range is also irreversibly diminished. Additionally, while the procedure may result in a higher pitch, many other important vocal components are not affected - creating an unbalanced voice that may not be acceptable to the speaker. Vocal training is typically required even after a glottoplasty is performed in order to modify these unbalanced components and create an authentic, feminine sound.
Achieving adequate pitch range for a feminine voice often does not require surgical intervention. Most clients are able to maintain a pitch that fits their goals without glottoplasty. I recommend that all individuals interested in this procedure seek the care of a physician to discuss their options.
The Components of Voice Feminization
Pitch
For most feminine transgender individuals, the goal is to avoid an artificially high pitch while maintaining a comfortable frequency within a feminine pitch range. While elevating pitch is often misunderstood as the primary component of feminization, achieving a natural and feminine voice is much more involved.
Resonance
The resonance - or brightness/darkness - of the sound is an important component in voice gender. By manipulating the spaces in the vocal tract, we can create overtones that brighten the voice, resulting in a more focused, feminine sound.
Melody
Intonation, the melody of speech, plays a significant role in how gender is perceived through voice. Feminine speech features a dynamic melody with increased variation in pitch. Learning to incorporate these patterns into your speech can enhance the femininity of your voice.
Speech Patterns and More
Beyond technical aspects, speech patterns and language use are also important. This includes using more expressive language, softer articulation, and varied pace of speech. Many small, faint components contribute to the gender of one's voice and can be refined in voice sessions.
